Pain Management in Swansea, IL
The Swansea commute along I-64 does not count as work in your job description. Your spine counts it, though not for the reason you have been told.
Swansea sits on the I-64 corridor about 40 minutes from our clinic with no traffic. If the drive hurts more than the job, read on. The evidence on sitting is more surprising than the pamphlets.

Does sitting cause low back pain?
Every back-pain handout blames the chair. The prospective evidence, which follows people forward in time, does not go along. A 2022 meta-analysis of longitudinal studies in adults found no consistent link between total sitting time and new low back pain. A 2021 review of sitting at work found only a modest association, with low certainty. What the research does show clearly is this: people who drive for a living report low back pain more than any other site. Sitting still is not the mechanism. Sitting still in a moving vehicle, for hours, with everything that comes with it, is closer.
Can driving long distances cause back pain?
A lumbar disc has no blood vessels of its own. Nutrients reach it by diffusion, a slow soak, and diffusion needs movement. An hour of stillness with road vibration coming up through the seat is the one posture that starves the disc and loads it at the same time. Second, the hip flexors shorten and the glutes switch off on a long drive. So when you climb out at the office, the facet joints take the lift your hips should have taken. Third, the metabolic piece. A drive-through breakfast eaten at the wheel spikes insulin. That spike primes every joint lining for inflammation before the workday starts. And the money driver is the commute itself. You trade time for housing, and the time comes out of sleep and movement, the two things that keep a spine quiet.
Under 7%
of US adults are metabolically healthy on the tighter cardiometabolic criteria applied to NHANES after 2021, down from under 12.2% on NHANES 2009–2016. Within this clinic’s chronic pain population, under 1%. Practice-reported figures from our own population, not trial outcomes, and individual results vary.
What the drive is actually loading
THE COMMUTER PATTERN
Low back stiffness that peaks getting out of the car, eases after ten minutes of walking, and returns with the drive home. Usually facet or disc, and the exam separates them.
THE PATTERN THAT IS NOT THE SPINE
Buttock pain on the driving side, with tingling down the leg that shows up only in the seat. It is often a hip or a sacroiliac joint loaded by one-legged pedal work. A spine injection does nothing for it.
THE SHORT LIST
New leg weakness, numbness in the saddle area, bladder or bowel change, fever, or pain after a crash on the interstate. These need imaging the same week, and a surgeon’s opinion where it is needed.
How does the exam find the source of the pain?
- Provocation, seated and standing. Pain that changes when you bend back points at the facets. Pain that runs below the knee with a straight-leg raise points at a nerve root.
- The hip and the sacroiliac joint are examined before any spine test is ordered, because in a driver both can pass for a disc.
- Bloodwork including fasting insulin, and a review of the imaging you already have. Nothing is injected on the first visit.
- A diagnostic block if the pattern justifies one, at a second visit, with a pain diary for the afternoon. No sedation is used, so most patients drive themselves home.
Where the restlessness helps
Dr. Gurpreet Singh Padda, MD, MBA, MHP will admit to a bad case of shiny-object syndrome. He once hired contractors to design a ground-based slingshot to launch payloads to orbit, before the physics killed it. That same restlessness made him refuse to stop at the disc. He kept going, to the insulin, the sleep and the schedule. It is why a Swansea back-pain visit includes a metabolic panel.
How can I relieve lower back pain caused by driving?
Behavior is half the plan, and each step comes with its reason. Two minutes of walking at the park-and-ride or the gas station restores disc diffusion and turns the glutes back on. Moving breakfast off the dashboard and dropping the acellular carbohydrate lowers the morning insulin surge. A fixed bedtime protects deep sleep, where the brain rebuilds its own pain brakes. A commuter who wakes at five and sleeps at midnight has already lost it. We offer Acceptance and Commitment Therapy in-house, with our own licensed pain-trained clinician, because bracing against the pain for ninety minutes a day is a habit that has to be unlearned, not medicated. And an hour alone in a car twice a day is isolation on a schedule. We treat it as a pain driver, not a lifestyle note.

Frequently asked questions
Is sitting really bad for my back?
Total sitting time has not been shown to cause new back pain in longitudinal studies. Stillness plus vibration plus lost sleep is a different matter. The sleep, movement and behavior page explains what to change first.
Could the pain from the drive be a disc?
It can be, especially when it runs below the knee and gets worse with coughing. The herniated disc page describes how a disc is confirmed, not assumed from a scan.
What does pain management do on a first visit?
Examine the spine, the hips and the sacroiliac joints, read your imaging with you, draw blood and write the plan. The what to expect page lays out the hour.
I drive for a living and my back is worse every year. Is that a work injury?
Whether cumulative driving strain qualifies as a claim is a question for an attorney or the Illinois Workers’ Compensation Commission; what the injured at work page covers is how we examine and document it either way.
What does an overworked lower back feel like after a long drive?
The commuter pattern is low back stiffness that peaks as you climb out of the car. It eases after about ten minutes of walking and comes back on the drive home. That is usually a facet joint or a disc. Buttock pain on the driving side, with tingling down the leg that only shows up in the seat, is more often the hip or the sacroiliac joint.
Will a lumbar support fix back pain from driving?
The seat is not the main problem. Total sitting time has not been tied to new back pain in long-term studies. What loads the spine is hours of stillness with road vibration, short hip flexors, and the sleep the commute costs. Two minutes of walking at the park-and-ride restores disc nutrition and switches the glutes back on. That is why the plan starts with movement and sleep, not a new chair.
Why does a back pain visit include bloodwork?
Because insulin sets how inflamed every joint lining is. A drive-through breakfast eaten at the wheel spikes insulin before the workday starts, and that primes the joints for inflammation. The first visit draws bloodwork including fasting insulin, along with the exam and a review of your imaging. So the plan treats the metabolic driver as well as the structure.
Can chronic pain be cured?
Some of it can, and the first visit exists to find out which kind you have. Pain from a structure that can be named and tested, such as a facet joint confirmed by a block, or a nerve kept on edge by high insulin and short sleep, can often be reversed, so the plan aims to reverse it. Pain from damage that cannot be undone is managed instead, and the target becomes the most function at the least exposure to drugs and procedures. Learn more: what a diagnostic nerve block tells you.

The structure gets named before the drive gets blamed
If you are coming from Swansea, the first visit finds out whether it is the facets, the disc, the hip or the sacroiliac joint, and what the commute is doing to each. You will not be told to buy a better chair and come back in six weeks.
