Pain management ยท Wood River, IL

The rotation at the Wood River refinery decides when you sleep, and when you sleep decides how much a disc is allowed to hurt.

Wood River is about 30 minutes from our clinic with no traffic. If you work rotating shifts along the refinery corridor, the schedule is part of the mechanism. The plan is built around it.

Does shift work cause back pain?

Rotating shifts do not only make you tired. They move your body clock, and the clock controls more than sleep. Cortisol peaks at the wrong hour. Insulin sensitivity falls when meals land at night. Inflammatory signals drift upward. A 2016 review in the BMJ of the health effects of shift work found links to metabolic disease and disturbed sleep. Both amplify pain before any disc is involved.

So the back pain of a refinery operator has at least three drivers. The first is mechanical. The climbing, the valve work and the long standing of a turnaround load the discs and facet joints. The second is biology. A body running on five hours of daytime sleep has less descending inhibition, the brake the brain puts on a pain signal at the spinal cord. It has more of the inflammatory chemistry that sharpens that signal. The third is money. Twelve-hour rotations exist because staffing them is cheaper, and the bill is paid in bodies. Treat the disc, ignore the rotation, and you have treated a third of the problem.

Does poor sleep make back pain worse?

A 2024 meta-analysis pooled prospective studies in adults with muscle and joint pain. It found that sleep problems predict later pain at least as strongly as pain predicts later sleep problems. A short night today raises tomorrow’s pain before anything changes in the spine. The mechanism review behind that points at the systems the shift schedule disturbs: opioid and dopamine signaling in the brain, and inflammation in the blood.

The result is blunt. An epidural, a facet ablation or a stimulator each buys a window. Spend the window on a broken sleep schedule and it closes early. You will be told that before the procedure.

What will the doctor ask a shift worker at the first visit?

Your rotation, and how many hours the swing costs you. Whether you snore, because untreated sleep apnea is common in shift workers and doubles as a pain problem. When you eat on nights, because a large meal of acellular carbohydrate at three in the morning makes an insulin spike at the point in the body clock least able to handle it. And whether there was a fall or a lift on a turnaround.

THE USUAL PICTURE

Mechanical low back pain, worse at the end of a set of nights and better on the first day off. It sits on a base of short sleep and rising blood sugar. Treatable, and the schedule is half the treatment.

THE PAIN THAT IS A SLEEP DISORDER

Widespread aching, morning headaches, sleep that does not refresh, and a partner who reports the snoring. The spine is not the main problem. A sleep study comes before any injection.

THE SHORT LIST THAT CANNOT WAIT FOR A SHIFT TO END

New leg weakness, numbness in the groin or saddle area, loss of bladder control, fever with back pain, or back pain after a fall from height. Same-week imaging, and a surgical opinion where needed.

What he got wrong

For twenty years Dr. Gurpreet Singh Padda, MD, MBA, MHP told patients with a slowly rising fasting glucose that the number was not too bad. In shift workers, it was the schedule writing itself into the bloodwork, and the spine was reading it. He was wrong. That is why this clinic draws a metabolic panel on the first visit from anywhere in the Metro East.

Under 7%

of US adults are metabolically healthy on the tighter cardiometabolic criteria applied to NHANES after 2021, down from under 12.2% on NHANES 2009–2016. Under 1% of this clinic’s chronic pain patients clear the same bar. Practice-reported figures from our own population, not trial outcomes, and individual results vary.

How can shift workers relieve back pain caused by work?

Behavior is not the aftercare here, and every instruction carries its mechanism. Keep an anchor sleep period fixed across the rotation. It gives the clock one point it can hold. Bright light at the start of a night shift and dark glasses on the drive home move melatonin to where it belongs. A caffeine cutoff six hours before sleep protects the deep sleep that restores the body’s pain brakes. The behavioral half, Acceptance and Commitment Therapy, comes from a licensed pain-trained clinician on our own staff. That is because a schedule you cannot change is what that therapy was built for. And rotation isolates people from everyone who keeps daytime hours. Isolation is a pain driver, and it is treated as one.

If the injury happened on the unit

Illinois lets an injured worker choose their own treating physician, subject to the two-choice rule in the Workers’ Compensation Act unless the employer runs an approved preferred provider program; the Illinois Workers’ Compensation Commission administers the claim. What that means for your case is a question for an attorney or the Commission. What we do is examine you, document it in a record that holds up, and treat the injury.

How far is the clinic from Wood River, IL?

From Wood River, IL-3 or IL-111 south to I-270, across the Chain of Rocks into Missouri, then a short run south on I-170 or Lindbergh and onto I-70. We hold early and late slots so a night-shift worker is not seen in the middle of their night. Free-flow times, no traffic:

Frequently asked questions

Can I be seen around a rotating shift?

Early and late slots are held for shift workers. Follow-up after a procedure can often be by video. Tell us your rotation when you get in touch.

Does one bad night really change pain the next day?

In prospective studies, a short night lowers the pain threshold the next day, before anything changes in the spine. The physiology is on the sleep, movement and behavior page.

I was hurt on a turnaround. Can I choose my own doctor in Illinois?

Illinois gives an injured worker a choice of physician within the two-choice rule, and the injured at work page explains how the exam and record are handled with a claim.

Will an epidural help if I cannot fix my sleep first?

An epidural steroid injection can still open a window for an inflamed nerve root. The sleep work is what keeps it open, so both happen.

Is shift work bad for your body?

It moves the body clock, and the clock runs more than sleep. On rotating shifts, cortisol peaks at the wrong hour. Insulin sensitivity drops when meals land at night. Inflammatory signals drift upward. A 2016 review in the BMJ linked shift work to metabolic disease and disturbed sleep. Both amplify pain before any disc is involved.

What does back pain from night shifts usually feel like?

The usual picture is mechanical low back pain that is worst at the end of a set of nights and better on the first day off. It sits on a base of short sleep and rising blood sugar. It is treatable, and the schedule is half of the treatment. So the plan is built around your rotation.

Could snoring be part of my back pain?

Yes. Untreated sleep apnea is common in shift workers and doubles as a pain problem. Widespread aching, morning headaches, sleep that never feels restful and a partner who hears the snoring all point that way. When they do, the spine is not the main problem, and a sleep study comes before any injection.
