Pain Management in Alton, IL

Thirty minutes from Alton over the Clark Bridge is short enough to build a pain diagnosis in stages instead of guessing it in one visit.

Alton sits about 30 minutes from our clinic off I-70 with no traffic. That is close enough to stage the workup: one question answered per visit, and no nerve treated on a guess.

Why the length of the drive decides how a diagnosis gets built

There is a quiet rule in pain medicine that nobody writes down. The farther a patient drives, the more gets done per visit. It feels kind. It is also how a wrong diagnosis gets a needle put in it. Alton is about 30 minutes from the clinic, close enough that we do not have to make that trade for Metro East patients.

Back pain that has lasted more than three months rarely has one cause. The first cause is mechanical. The facet joints at the back of each lumbar segment take more load as the discs in front of them lose height. The second is biology. With insulin resistance, the joint lining runs an inflammatory program of its own, so the same load hurts more. The third is money. A clinic paid per procedure has every reason to treat the first likely structure instead of confirming it. Staging the visits takes that incentive out of the room.

Can an MRI show facet joint pain?

The facet joint does not announce itself on an MRI. Arthritic-looking facets are common in people with no pain, and clean-looking facets can hurt. The only test is to numb the two small nerves that carry the joint’s signal, the medial branches. Then watch what your pain does for the next few hours. A sharp drop that returns when the anesthetic wears off is a positive block. We do it twice, on separate days. That is because one block is fooled by placebo and by anesthetic spread.

THE PATTERN THAT USUALLY BLOCKS POSITIVE

Pain across the low back, not down the leg. Worse leaning back or twisting. Worse first thing after sitting. Tender right over the joint line. An hour at a workbench or a kitchen counter makes it worse; walking often helps.

THE PATTERNS THAT IMITATE IT

Sacroiliac pain sits lower, below the belt line and off to one side. Rolling over in bed brings it on. Hip arthritis spreads into the groin. Both get treated as facet pain for years when nobody tests them.

THE SHORT LIST THAT SKIPS THE STAGES

New leg weakness, numbness in the saddle area, loss of bladder or bowel control, fever with back pain, or unexplained weight loss. Same-week imaging and, where needed, a surgical opinion.

Does radiofrequency ablation work for back pain?

The evidence is truly mixed. The Dutch MINT trials, published in 2017, randomized 681 patients with chronic low back pain. They got radiofrequency denervation plus exercise, or exercise alone. The trials found no meaningful difference. But patients had entered on a single block with a 50% relief threshold, and technique varied across sites. A 2020 multispecialty guideline read that as a lesson about selection. A 2023 pragmatic trial chose patients more carefully. It found cooled radiofrequency beat steroid injected into the joint at three months, with the gain holding through twelve. The practice’s position: the procedure works when the diagnosis was earned and fails when it was assumed.

What happens at each visit from Alton?

  1. First visit: examination, your imaging read with you, bloodwork, a written plan. Nothing is injected on day one.
  2. Second visit: the first medial branch block, twenty minutes under fluoroscopy, then a pain diary that afternoon. No sedation is used, so most patients drive themselves home.
  3. Third visit: the confirmatory block, only if the first worked. It uses a different anesthetic, so the length of relief tells us something new.
  4. Fourth visit: radiofrequency ablation of the nerves that tested positive twice.
  5. Follow-up at two weeks, by video where that is clinically sound.

Four short trips is less road than one long day that ends with the wrong structure treated. From the riverfront the Clark Bridge carries US-67 over the Mississippi; on the Missouri side it becomes Lindbergh Boulevard and runs down to I-70, one exit from Woodson Rd.

What he got wrong for twenty years

Dr. Gurpreet Singh Padda, MD, MBA, MHP spent two decades telling patients with a fasting glucose in the high nineties that it was not too bad. He was wrong, and their facet joints were the evidence. Those joints sat inside a body making its own inflammation, where no local injection holds for long. That is why the first Alton visit includes a metabolic panel.

Under 7%

of US adults are metabolically healthy on the tighter cardiometabolic criteria applied to NHANES after 2021, down from under 12.2% on NHANES 2009–2016. Among this clinic’s chronic pain patients the figure is under 1%. Practice-reported figures from our own population, not trial outcomes, and individual results vary.

Half of the plan is not a needle

An ablation buys months. What you do with them is 40 to 50% of the protocol, and every piece has a mechanism. Daily walking rebuilds the multifidus muscles that unload the facets. Removing acellular carbohydrate lowers insulin. Lower insulin turns down the cytokines (inflammatory signals) the joint lining was making. A short night lowers the next day’s pain threshold, so sleep is prescribed. A licensed clinician trained in pain runs Acceptance and Commitment Therapy in-house here. That is because retraining the fear of movement is what keeps the muscles working once the nerve is quiet. Isolation amplifies pain; the riverfront on foot is part of the plan.

How far is the clinic from Alton and nearby towns?

Free-flow times from the clinic with no traffic. Weekday afternoons on the bridges add to all of them.

Frequently asked questions

How many visits does a facet diagnosis take from Alton?

Three before anything lasting is done: an exam, then two separate medial branch blocks. The ablation is a fourth, only for patients who tested positive twice.

Can the injection just be done on the first visit?

The first visit is the exam, the imaging review and the bloodwork, as set out on the what to expect page. A block done before the exam is aimed at a guess.

How long does the ablation last?

Six to twelve months is typical, and the medial branches regrow. The radiofrequency ablation page explains why regrowth is expected and not a failure.

What if the blocks do not relieve my pain?

A negative block takes the facet joint off the list. It sends us to the structures that mimic it, most often the sacroiliac joint or the hip.

Is it worth getting a medial branch block?

It is the only test that proves a facet joint is the source of your pain. An MRI cannot. Arthritic-looking facets are common in people with no pain, and clean-looking facets can hurt. The block numbs the two small nerves that carry the joint’s signal. A sharp drop in pain that returns as the anesthetic wears off is a positive result. Without that proof, an ablation is aimed at a guess.

How long does a facet block injection last?

A diagnostic medial branch block is a numbing medicine, so the relief is meant to last only the next few hours. That short window is the test. You keep a pain diary that afternoon. The confirmatory block on a separate day uses a different anesthetic, so the length of relief tells us something new. The lasting treatment is the radiofrequency ablation that follows two positive blocks.

What is the best treatment for facet joint pain?

For a facet joint that has tested positive twice, radiofrequency ablation of its medial branch nerves quiets the joint for months. It works when the diagnosis was earned and fails when it was assumed. That is why the blocks come first. The months it buys go to the other half of the plan: daily walking to rebuild the multifidus muscles, less acellular carbohydrate, sleep, and Acceptance and Commitment Therapy.
