Sciatica treatment · Metro East, IL
For sciatica, the bridge is the first test. How long you last in the driver’s seat before the leg lights up is part of the diagnosis.
Sciatica is a nerve root under chemical and mechanical pressure at the same time. How long you can sit, how far you can walk and what your blood sugar is doing to the nerve decide the timeline. They matter more than the size of the disc.

Why does driving make sciatica worse?
Sitting bends the lumbar spine forward and shifts load onto the front of the disc. That pushes the soft center backward, toward the exact spot where the nerve root leaves the canal. Standing and walking unload it. That is why a person with a disc-driven root problem can walk the dog but cannot survive the drive to work. The opposite pattern, easy in the car and miserable on foot, points to stenosis instead. The crossing from the Metro East is a provocation test we did not have to design.
So we want numbers before you arrive. From Edwardsville our door is about 40 minutes with the bridge clear and no traffic; from Granite City about 25; from Belleville about 40. If the leg starts before Poplar Street, you can sit for less than twenty minutes, and that tells us about the root. If you can sit the whole way and the leg starts in the parking lot, that is a different structure.
What triggers sciatica pain?
Two things, and the second is the one nobody draws on the whiteboard. The first is mechanical: a disc fragment, a thickened ligament or a bony spur narrowing the exit for the root. The second is chemical. The center of the disc has been walled off from your bloodstream since childhood. So when it leaks, your immune system treats it as foreign and floods the root with inflammatory signals. An inflamed root fires at a fraction of the pressure a quiet root can take. That is why a modest-looking herniation can cause a screaming leg. It is also why the same fragment can go quiet without changing size.
Why does sciatica last longer for some people?
Two people with identical scans do not get identical sciatica. The difference shows up in a blood draw. Insulin resistance thickens the endplate the disc feeds through. It glycates the disc (binds sugar to it) so it holds less water. It raises the inflammatory tone the root has to fight. Alpantaki and colleagues found disc degeneration tracking diabetes across most studies they reviewed. Smoking clamps the small vessels that supply the root.
33%
In four Finnish cohorts totaling 34,589 adults followed for 12 to 30 years, walking or cycling to work cut the risk of hospitalization for sciatica by a third, independent of body weight, while current smoking raised it by 33% and obesity by 36%. Shiri and colleagues, 2017.
Then the part that is not biology. The forklift seat at the Gateway Commerce Center warehouses outside Edwardsville is ten hours of vibration, bent forward. The refinery rotation in the Wood River corridor puts you on nights. One short night lowers the pain threshold and raises inflammatory cytokines in the blood by the next afternoon. The nerve does not know which of those is the cause; it feels the sum.
How long does it take for sciatica to calm down?
Most of it heals. Peul and the Leiden group randomized 283 Dutch patients with six to twelve weeks of severe sciatica. They got early microdiscectomy, or conservative care with surgery only if needed. Early surgery relieved leg pain faster. At one year, 95% of both groups reported recovery. The trial left out cauda equina symptoms, prior spine surgery and severe other illness. But the shape holds. The nerve usually recovers. The job is to make the window bearable and to notice when you are off that curve.
THE USUAL COURSE
Pain below the knee in one leg. Worse seated, easier walking. Numbness that maps to one root. No weakness that keeps getting worse. This improves week over week. The plan is movement, sleep and the blood sugar. Where pain blocks those three, a transforaminal epidural quiets the chemical side while the biology works.
NOT ON THE CURVE
Pain that has not moved at six to eight weeks. A foot that is measurably weaker but stable. Or two possible levels on the scan. A diagnostic transforaminal block settles which root is to blame. Surgery is a real option for sciatica when weakness gets worse, or when pain outlasts a well-placed injection and a corrected terrain. This practice performs microdiscectomy then.
DO NOT DRIVE HERE. GO TO AN EMERGENCY ROOM
New trouble starting or holding urine, numbness where you sit on a saddle, both legs weakening at once, a foot that suddenly drops, fever with the back pain, or new back pain with a cancer history or unexplained weight loss. Read when it is an emergency and act on it.
Care built for a patient who lives an hour away
The first visit is an examination, a history someone writes down, a review of any imaging you already have, and bloodwork. Nothing gets injected on day one because nothing has been diagnosed on day one. Procedures use no sedation, so most patients drive themselves home; if a leg feels weak or numb, someone else drives until that wears off. Follow-up that does not need hands on you happens by telehealth where clinically appropriate. If the leg pain started with a lift at work in Illinois, the Illinois Workers’ Compensation Commission administers the claim and 820 ILCS 305/8(a), the two-choice rule, generally leaves the choice of physician with the injured worker; that is a description, not legal advice. See injured at work.
The 40 to 50 percent of the plan that is not a needle
Walking is a treatment. Each stride loads and unloads the disc, which is its only pump for moving nutrients across the endplate. It also milks the congested (backed-up) veins around the root. Sleep is a treatment. The nervous system resets its pain volume overnight. A week of five-hour nights turns up the volume on every signal the root sends. Food is a treatment, because lowering the insulin lowers the inflammatory tone the root sits in. And the fear of moving is treated in this building. A licensed, pain-trained behavioral clinician uses Acceptance and Commitment Therapy. That is because a leg you are afraid to use becomes a leg you stop using, and disuse keeps pain alive after the fragment is gone.
Frequently asked questions
Do I need a referral to be seen for sciatica?
No. Call, text or book online and you are on the schedule, no referral needed. Some insurance plans want one on file before paying, so check your card. If an epidural becomes part of the plan, driving home after a spine injection covers the ride back over the river.
Does sciatica go away on its own?
Most of it does, over weeks to a few months. That is because your own immune system breaks down the herniation that started it. The work is making that window livable and catching the minority who are not improving. Herniated disc explains how the body clears it.
Why does sitting make sciatica worse and walking make it better?
Sitting bends the spine and pushes disc material toward the root. Walking unloads it and pumps blood out of the veins around the nerve. If your pattern is reversed, the more likely structure is the narrowed canal on the spinal stenosis page.
Can an epidural injection fix sciatica?
It treats the inflammatory half of the problem, not the fragment. So it is a way through a period that would otherwise be unbearable, not a repair. The epidural steroid injection page covers routes, limits and what steroid does to blood sugar.
If my sciatica started while lifting at work in Illinois, does that change anything?
It changes who administers the claim and how carefully function is documented from the first visit, and in Illinois it usually leaves the choice of physician with you. The two-choice rule is explained on injured at work.
When is sciatica an emergency?
When bladder or bowel control changes, the saddle area goes numb, both legs weaken together or a foot drops suddenly. The full list is on when it is an emergency. It means an emergency room, not us.
How do you fix sciatic nerve pain?
Most sciatica heals as the body clears the disc material, so the plan supports that healing. Walking pumps nutrients into the disc and blood out of the swollen veins around the nerve. Sleep turns the pain volume down. Lower insulin calms the inflammation around the root. When pain blocks all three, a transforaminal epidural quiets the chemical side. Microdiscectomy comes in when weakness gets worse or pain outlasts a well-placed injection.
What is the worst thing you can do for sciatica?
Stop using the leg. A leg you are afraid to move becomes a leg you stop moving. That disuse keeps pain going after the disc fragment is gone. Long stretches in a seat also load the disc toward the nerve. A week of five-hour nights turns up every signal the root sends. Keep walking, protect your sleep, and treat the fear of moving as part of the problem.
What can be mistaken for sciatica?
Spinal stenosis is the common look-alike. Disc-driven sciatica is worse sitting and easier walking. If the leg is fine in the car and miserable on foot, a narrowed canal is the more likely cause. Sometimes the scan shows two levels that could be to blame. A diagnostic transforaminal block settles which nerve root is actually causing the pain, before anyone plans a bigger step.

You will leave the first visit knowing which structure is doing this and what the next four weeks look like
One crossing gets you an examination, a diagnosis you can repeat back to us, and a plan sequenced for someone who cannot come in every week. If a needle is part of it, you will know its job and its endpoint before you agree to it.
