Pain management ยท Glen Carbon, IL
The trail through Glen Carbon is part of the prescription. The knee that stopped you using it is the diagnosis we start with.
Knee, hip and back pain care for Glen Carbon, about 35 minutes free-flow from the clinic. It is built to get you back onto the Heritage and Nickel Plate trails, not off them.

Eleven miles of rail-trail run through this village
Glen Carbon sits between Edwardsville to the north and Maryville to the south, with I-270 crossing above its center. Its defining feature is not a highway. The Ronald J. Foster Sr. Heritage Trail follows the old Illinois Central right-of-way for eleven miles through the village. It runs past the covered bridge by the library on Main Street. It was one of the first rails-to-trails conversions in the country. The Nickel Plate Trail adds another fifteen. People here walk and ride. This page is written for the one who has stopped, because a knee, a hip or a low back made the trail hurt more than it helped.
Stopping feels right, and it is wrong. The reasons are physiological (how the body works), not about willpower.
Is it better to walk or rest with knee arthritis?
Articular cartilage (the smooth lining of the joint) has no blood supply. It is fed by joint fluid, and the fluid moves only when the joint is pressed and released. A knee that is rested is a knee that is being starved. The thigh and hip muscles that unload the joint with every step waste within weeks of disuse. That sarcopenic wasting lands the full body weight on the cartilage the muscle was protecting. And the inflammation is not confined to the joint. Fat tissue in a metabolically inflamed body puts out the same inflammatory signals that degrade cartilage. That is why knee osteoarthritis tracks insulin resistance, even in joints that were never overloaded. Add the social cost: the walking partner who now walks alone, and the sensitized nervous system that follows isolation. Now the rested knee has three drivers pushing it the wrong way at once.
The Cochrane reviews are clear. Land-based exercise brings moderate drops in pain and gains in function in knee osteoarthritis. The benefits last for months after supervised sessions end. The hip review reaches the same conclusion. Stationary cycling is the closest lab match to what the trail offers. In the pooled trials it reduces pain and improves sport function in knee osteoarthritis. The trail is not what you return to after treatment. It is the treatment. The procedures exist to make it possible.
What if my knee hurts too much to exercise?
Which brings us to the problem. Exercise is the therapy with the best evidence, and a knee that hurts too much to move cannot take it. That is exactly the gap an interventional procedure is for. It should be framed that way, not as a repair.
A DIAGNOSTIC BLOCK FIRST
We numb the genicular nerves around the knee under imaging. In one afternoon, that tells us whether the pain is coming from the joint. If it is, a genicular block can be followed by radiofrequency ablation of those nerves. That quiets the joint for months, long enough to rebuild the thigh muscle on the trail. The joint is not changed. What you can do with it is.
INTO THE JOINT ITSELF
An image-guided joint injection has its place for an acutely inflamed knee or hip. But steroid inside a joint raises blood glucose for days, and we do not run it on a schedule. How many is a clinical decision, made against what the last one achieved.
NOT A KNEE AT ALL
Pain in the front of the thigh with a normal knee exam, or buttock pain that runs below the knee, is often a lumbar root or a sacroiliac joint wearing a knee costume. The exam finds it. The knee pain page explains how.
Is my pain coming from the hip, the SI joint or the back?
True hip joint pain lives in the groin and the front of the thigh. Rolling the leg inward brings it on. Buttock pain that hurts when you roll over in bed and sit on one side points to the sacroiliac joint, an inch to the side of the spine. Pain that runs down the outer leg to the ankle is a lumbar root. Three structures, one word, and the wrong one gets injected all the time. The hip pain page sets out how the exam separates them. The hip capsule sits deep, so when it is the culprit, imaging, not feel, is how we reach it.
Thirty-five minutes, with I-270 at your door
The clinic is about 25.1 miles from the center of the village and roughly 35 minutes at free-flow speeds with no traffic modeled. The route is the one you already know. Take I-270 from exit 9 or 12, west across the Chain of Rocks. Then take I-170 or Lindbergh south to I-70 at Woodson Road, next to Lambert Airport. No sedation is used for a genicular block or a joint injection; you stay awake under local anesthetic, and most patients drive themselves home. Normal activity, including work, resumes within two to four hours. If a leg feels weak or numb, have someone else drive until that wears off.
What we do before a replacement
We treat the knee now, rather than telling you to wait for a replacement as if nothing can be done until then. A joint that needs muscle around it more than steroid inside it gets the strength work, not a standing series of injections. This practice handles the interval before a replacement, and the pain that sometimes lasts after one; the operation itself is done by an orthopedic surgeon elsewhere. Acceptance and Commitment Therapy happens in this building, from a licensed clinician trained in pain. That is because a person who has learned to fear the trail needs that unlearned with the same seriousness as the nerve block. And the bloodwork we order at the first visit is there to find the metabolic inflammation that is degrading the cartilage from the inside.
Frequently asked questions
Should I stop walking the trail until my knee is treated?
Shorten the distance and keep going. Cartilage is fed by movement, and the thigh muscles waste within weeks of rest. If the pain will not allow even a short loop, that is the signal for a diagnostic block. The genicular nerve block page explains it.
Is a genicular nerve block the same as a cortisone shot in the knee?
A genicular block numbs the small nerves around the joint from outside it, and it works as a test. A cortisone shot goes into the joint space. They answer different questions. The knee pain page sets out when each one is the right tool.
My hip pain is in the buttock. Is that the hip?
Buttock pain that hurts when you roll over in bed usually points to the sacroiliac joint, not the hip capsule. Pain running below the knee points to a lumbar root. The exam sorts them, and the hip pain page explains the difference.
How far is the clinic from Glen Carbon?
About 25 miles and roughly 35 minutes at free-flow speeds, I-270 west over the Chain of Rocks to I-170 and I-70 at Woodson Rd. Parking and arrival details are on the contact page.
Do you do knee replacements?
Joint replacement is an orthopedic operation done elsewhere. This practice treats the interval before a replacement is needed, and the pain that sometimes lasts after one. The second problem is described in when injections stop working.
What is the best exercise for knee arthritis?
Land-based exercise has the best evidence. Cochrane reviews found it reduces pain and improves function in knee arthritis, with benefits lasting months after supervised sessions end. The hip review agrees. Stationary cycling reduces pain in the pooled trials too. In Glen Carbon, walking or riding the Heritage and Nickel Plate trails is that exercise. The procedures exist to make it possible, not to replace it.
What should you not do with knee arthritis?
Do not stop moving. Cartilage is fed only when the joint is pressed and released. The thigh and hip muscles that protect it waste within weeks of rest. Do not accept a standing series of steroid injections, since steroid inside a joint raises blood glucose for days. And do not wait for a replacement as if nothing can be done until then.
What can be mistaken for knee pain?
Pain in the front of the thigh with a normal knee exam, or buttock pain that runs below the knee, often comes from a lumbar nerve root or the sacroiliac joint, not the knee. That is why the exam checks the back and pelvis too. A diagnostic block of the genicular nerves then shows in an afternoon whether the pain really comes from the knee joint.
